(hereinafter referred to as the “Data Subject”)

I hereby grant RS Tennis s.r.o., ID No. 14213532, with registered office at Žitavského 539, Zbraslav, 156 00 Prague 5 Czech Republic, registered in the Commercial Register maintained by the Municipal Court in Prague, Section C, Insert 361711 (hereinafter referred to as the “Controller“) consent to the processing of my personal data, namely data about my health condition, for the purpose of proper provision of the services of a personal tennis trainer ordered by me.

The Controller may process personal data based on this consent for the duration of the contractual relationship with the Controller, i.e. for the duration of the provision of the ordered services.

I declare that the personal data provided is correct and in the event of any change in the personal data provided, I will notify the Controller of such change. I expressly consent to the processing of my personal data under the conditions set out above. I declare that I have been informed of my rights in relation to the processing of my personal data.

Under the conditions set out in Articles 15 to 22 of the GDPR, the Data Subject has the right to:

– request free access to their personal data processed by the Controller,

– request a copy of the personal data,

– request the rectification of inaccurate personal data,

– request the erasure of personal data processed by the Controller if they are no longer necessary for the purpose provided, if the Data Subject withdraws consent or objects to processing, or if they are processed unlawfully by the Controller,

– request the restriction of the processing of their personal data in the cases provided for in Article 18 GDPR,

– data portability.

The Data Subject has the right to lodge a complaint against the processing with the supervisory authority, which is the Office for Personal Data Protection, Czech Republic.

The Data Subject has the right to withdraw this consent to the processing of personal data at any time via email address [email protected]. The withdrawal of consent shall not affect the lawfulness of processing based on consent before its withdrawal. The Data Subject acknowledges that in the event of failure to provide or withdrawal of this consent to the processing of personal data, it will no longer be possible to provide the Data Subject with the ordered services, due to the impossibility of drawing up an individual training plan.
